※If we are unable to reach you by phone, we may contact you by email to confirm the date and time. Please note that if we are unable to contact you within 24 hours from the date of application (form submission date), your application will be automatically.

  • ・Asymptomatic, non-coherent contacts are eligible for this test, and this test cannot be performed on patients suspected of being infected with COVID-19.
  • ・If you have subjective symptoms, please contact a public health center and have a PCR test covered by public insurance.
  • ・In some areas, the test results may not be available until the following two months.
